charge that will need to be signed (must be a live signature) prior to attending any virtual meetings.

Club Meeting Attendance Requirements
Prior to attending any in person 4-H meetings meetings/practices a
COVID 19-Screening Form must be completed, signed (must be a live signature) and turned into the adult in charge.
This form must be completed for every in person event. Please watch for information from club managers and coaches for their process
for collecting these forms.
