Please remember to submit your child's eligibility form for the 4-H practice show (CCLS show) on December 19 by Friday, December 17. We only need it for academic reasons since it is on a weekend and the start of the holidays.
We like to have the form sent/brought back to us at least 1 week before the event in case there is a problem with getting it from the school.
You may fax, email, or bring in the completed form to us. Fax, 830-620-3446, orIf we do not have proof of eligibility prior to the event your child is entered in, the event host will be contacted and the child will not be allowed to compete.
The form is not available on our website nor on 4honline. We will email the form to members registered on 4-H Online or you may contact the office for the form. We will not accept forms from previous years.
